1K a year 6 days ago 6 days ago 6 days ago The Social Worker Memory Support Coordinator requires that 50% of total work hours are scheduled to provide care and services as the Social Worker and 50% as the Memory Support Coordinator.
The Memory Support Coordinator is responsible for coordinating all memory care services including supervision and direction of memory care neighborhood team members.
Qualifications:
Graduate of a CSWE accredited bachelor's degree required Master's degree preferred One-year experience of supervised work in a health care setting required Ability to assess and effectively communicate with residents Minimum of 1-year supervisory experience preferred Experience developing, supervising and facilitating specialized dementia and/or memory care programs preferred Responsibilities and Expectations:
Meets with resident/family upon admission to assess and plan to meet psychosocial needs.
Coordinates discharge planning process and arranges for resident services after discharge.
Actively participates in the Interdisciplinary Care Planning Conferences.
Completes appropriate documentation upon admission, transfer, discharge, and completes ongoing progress notes.
Coaches and enables other team members to respond to resident's memory care support, emotional and psychosocial needs.
Demonstrated knowledgeable of disease process, recognizing memory associated changes, interpreting how changes may affect function and identifying strategies to maintain abilities.
Responsible for assuring residents rights are met.
Orients residents and families and assists in adjustment through program involvement.
Conducts periodic memory support groups for families, team members and outside stakeholders.
Completes and maintains a memory support history and assessment for each resident upon admission and ongoing updates as needed.
May assist the Marketing or Admissions team with networking, providing tours, lecturing and attending related conferences or events.
Maintains a current community resource file for memory support education and community resources.
Designs and conducts formal and informal education for residents, family members and team members.

Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
